13 Suspect bird flu in Indonesia. 3 dead

3 dead in feared bird flu outbreak in Indonesia: officials
August 6, 2008 Wednesday
http://www.straitstimes.com/La…MEDAN (Indonesia) - THREE people have died and 13 have been admitted to hospital with symptoms of bird flu in Indonesia, a nurse treating the patients said on Wednesday.

Officials and residents in Asahan district of North Sumatra province said villagers began showing symptoms of avian flu after a large number of chickens died suddenly last week.

The nurse at Asahan district’s Kisaran hospital said three people had died after suffering bird flu-like symptoms in Air Batu village.

‘According to residents there, a number of chickens died suddenly last week followed by several pigeons. Days later, three people died with the same ailments,’ the nurse, Mariana, said.

Another 13 people had been admitted to the hospital with ‘high temperatures and respiratory problems,’ she said.

Two of these - a baby boy and a seven-year-old girl - were transferred early Wednesday to a bird flu isolation unit at Adam Malik hospital in the provincial capital of Medan, officials said.

Adam Malik hospital spokesman Sinar Ginting confirmed that blood samples from the two children were sent Wednesday to a health ministry laboratory in Jakarta for analysis.

‘We are now waiting for the result,’ he said.

The father of the baby boy, Slamet Riadi, said a lot of poultry had died in the village a week ago. His baby developed a high fever and respiratory problems shortly afterward.

A spokesman for the health ministry could not be reached for comment.

The ministry, which has stopped giving regular bird flu updates, announced earlier this week.

The virus is alive and thriving.

Recombinomics Commentary

Recombinomics Commentary August 6, 2008Three people have died and 13 have been admitted to hospital with symptoms of bird flu in Indonesia, a nurse treating the patients said Wednesday.

On Wednesday at dawn struck their 02,00 arrived. The two patients consisted of a male child and the woman, said public relations of RSUP Adam Malik of the Ginting rays, on Wednesday (6/ when being encountered by the reporter.

The official of the Intensive Care Unit (UGD) RSUD Abdul Manan, Ana, said, eleven other patients on behalf of RAPPING (5), IP (6), Spr (33), DY (5), the US (39), M. (32), D (32) Sdr (39), RM (Cool, and D (2).

The “patient D was the last patient who entered this morning (on Wednesday 6/Cool,” said Ana.

The above comments and translation describes three suspect fatal H5N1 infected patients as well as 13 more who are hospitalized at the Adam Malik hospital in Medan in North Sumatra.

This large number of suspect patients is cause for concern. The largest confirmed H5N1 outbreak in Indonesia was also in North Sumatra. (Snip) (Karo cluster) The large number of adults in the above list suggests the cases represent multiple families.

(Snip) Although Indonesia has acknowledged two lab confirmed cases in Tangerang last month, WHO has yet to publish updates on these patients. Similarly, the reporting of the cases preceding the cases last week would reported weeks after confirmation in violation of IHR regulations. The earlier cases were members of clusters, which Indonesia failed to confirm and WHO failed to acknowledge.

WHO’s continuing failure to report these clusters in Indonesia remains a cause for concern. Details on this large cluster and an explanation from WHO on its glaring oversight failures would be useful. http://www.recombinomics.com/N…

I get on rants sometimes when my frustration level goes through the roof. I believe at this point there is a cover up in Indonesia. Here's my rant:

Something still isn't right with this picture.
WHO comes in on a positive case. Ironically enough, all the villagers tested, turned out to be negative. 3 more villagers show signs of bf.

It is maintained that there are not any H5N1 cases. They start saying "Dengue." They won't speculate with in reason what is infecting these people with flu like symptoms.

What lenght would WHO go to to cover up human clusters of H5N1? Please, someone tell me. They ARE carrying on as if they are dealing with something severe. After all, suspect H5N1 clusters are rare. I don't believe it was the posive case that got WHO running, but "the report of a suspect H5N1 cluster. There are individual postive cases reported quiet frequently, and this kind of action isn't taken on the village the person lived in. Villagers are questioned and checked, not tested, and probably left with some Tamiflu.

I find it odd that the story has now changed since first reported and WHO is now in charge. Those villagers were frightened about something, and it wasn't ordinary flu, or Dengue. What would make those villagers panic? What does
WHO and other agencies have to do to stop their panic, and that of others reading about this? (throw out a PR blanket perhaps)

To me, their answers don't make any sense to the questions.
Or something like that. ;-)

The Tired Bird Flu loss of Rp4,3 Triliun By editorial staff of 04/09/2008 @ 02:44

Jakarta, on Thursday - the Spreading of the plague of bird flu in Indonesia caused the loss that was very big from the aspect of economics. The newest data that was launched by the Nasional Pengendalian Committee of Bird Flu and Menghadapi Pandemi Influenza Preparedness (Komnas FBPI) mentioned, the economic loss resulting from bird flu since 2004 till June 2008 reached Rp.4,3 trillion. "From 2004 to June 2008, bird flu has caused the Rp sacrifice 4.3 trillion and that not only from one because, but many because," said the Chief Executive of the Komnas FBPI Daily, Bayu Krisnamurthi, on Thursday (4/9), when opening Workshop Tantangan Penelitian Avian Influenza in the LIPI Building, Jakarta.

Bayu explained the so big value emerged after the calculation of various factors beginning with the death of the chicken, the decline in the request from consumption of the household, the decline in consumption of the chicken direstoran and other fees that must be paid by the breeder and the government in controlling bird flu and the influence towards the other sector. However this figure not yet including the loss figure resulting from the loss and the decline of the work opportunity asupan protein to the community. The problem of bird flu, continued Bayu, at this time indeed still became the serious problem for Indonesia. Despite tren in the world showed bird flu began to be able to be overcome, but in several last years, the endemic area of bird flu to the poultry in Indonesia was increasingly old increasingly wide.

The data that was expressed by Komnas FBPI showed, the loss that reached trillions that was not free from increasingly expanded him the area of the distribution of the endemic area of bird flu that attacked the poultry in Indonesia. "From 2003 to 2007, his area was increasingly wide." Now, practical almost all the province became the endemic area. Recorded was 31 from 33 provinces, although not all the regency in each affected province, he said. The bird flu threat for humankind personally, said bayu, still will continue shadowed remembered consumption of protein from animal meat in Indonesia still stayed high. Learned from the data last year for example the Indonesian community was recorded mengonsumsi around 1.2 billion chickens, approximately 1.5 million cattle and the water buffalo, 1.3 million goats and 1,5 million ton the fish. "Things if when rotten just a few will be poison." So the illness threat that came from the animal was real. That was real. Zoonotic disease that nyatam not something that was in the laboratory but real and became part of the life of the community, ujarnya./kompas.com

The socialisation of Bird Flu as Social Responsibility the Company
http://www.infeksi.com/newsdetail.php?lng=in&doc=2441

By editorial staff of 25/09/2008 @ 23:33

The bird flu illness (FB), still spread from the poultry to humankind. In fact in many Asian countries, Europe and Africa, the FB pandemic to the poultry has happened. With increasingly the frequency infected humankind, it was worried about the FB virus that Highly Pathogenic Avian Influenza (HPAI-H5N1) bermutasi became the virus that spread between humankind. When this happened, then the big disaster could not be evaded with the number of deaths and pain that many as well as the social disorder economics, the sadness as well as humankind's suffering. Therefore, all the sides including the company multi national was asked to have the FB control strategy and the Influenza Pandemic. Moreover the socialisation of the FB control to the employee and the environment him became part of social responsibility the company or Corparate Social Responsibility (CSR).

Was like this the statement of Menkes Dr. Dr. Siti Fadilah Supari, Sp.JP (K) opposite 100 owners of the company (the CEO) from various kinds of the efforts field, when opening "Sosialisasi Pengendalian FB and Menghadapi Pandemi Influenza Preparedness in the company's environment/industri" in Jakarta on September 23 2008. The Menkes concern was based by the past experience when the Spanish flu pandemic in 1918 happening that caused the death 40-50 million people all over the world in relatively short time. According to Menkes, the government was very serious in the FB control in Indonesia. Through the No. Presidential Regulation 7 in 2006 formed the Nasional Pengendalian FB Commission and Mengdadapi Pandemi Influenza Preparedness (Komnas FBPI) in order to coordinates FB control efforts nationally. The president also issued Inpres No. 1 in 2007 about the Handling and the Control of the FB virus, to press the number of FB sufferers to humankind. "Karena that, unwise when we were not on alert anticipated the threat of the influenza pandemic that will come.

The prevention of the emergence of the influenza pandemic in the period that will come globally to be striven for. If these efforts later evidently was not successful, then the impact of the pandemic must be able to be pressed as on a small scale as possible with preparedness that terencana and was tested rapi", said Dr. Siti Fadilah. The FB control and Menghadapi Pandemi Influenza Preparedness in the environment perusa-haan/the industry was the integral part of national preparedness and global, must be planned and diujicoba neatly and thoroughly. If this was not done, was difficult the impact of the pandemic was made as on a small scale as possible in the company's environment/the industry. This meant his impact on the wider community will stay very big and was not controlled. In fact, episenter this pandemic possibly began to happen in the certain part in the world, said Menkes. The virus H5N1 the first time being detected to the poultry in August 2003 and the sub-type infection H5N1 to humankind the first time it was confirmed in Indonesia in July 2005

The FB challenge increasingly increased both in the national level and in the regional and global level. The case of bird flu has to humankind been detected in many countries that is Azerbaijan, Bangladesh, Cambodia, China, Djibouti, Egypt, Indonesia, Iraq, Laos, Myanmar, Nigeria, Pakistan, Thailand, Turkey and Viet Nam. In the national level, the number of endemic provinces bird flu to the poultry really has increased so as only two provinces that still was free FB that is Gorontalo and the North Moluccas. The Luar Biasa incident (KLB) FB to the poultry still happened in a sporadik manner in various areas, added Menkes. Menkes stated, FB was the relatively new illness. Because of that still needed the study and the research from various experts like epidemiology, clinical, diagnostics, immunology and virology. For example, the early detection and FB beginning medical treatment in the Community Health Centre were needed by some kit fast diagnostics (rapid test) with the level of the sensitivity and spesifisitas high. So also in the aspect of the prevention, the FB vaccine for humankind still in the process of the producer's research of the vaccine and the experts

Total investment in poultry livestock breeding reached 35 Billion American dollar with the circulation of money reached US$*30 Milyar per the year. The livestock breeding industry jugai absorbed 10 million manpower. Whereas the total poultry reached 1.3 Billion tails, where 20% among them was kept behind the house (backyard farm) by 30 million households in Indonesia. Therefore, the control of the FB problem in Indonesia might not be done in a manner sem-barangan, but must terencana and was implemented thoroughly as well as neat. Like in the saying "seperti pulled hair from flour; flour should not be hoarsest and hair lest putus", said Menkes. Menkes stressed, Indonesia has had the Stratejik Nasional Plan for the FB control and Pandemi Influenza Preparedness (National Strategic Plan for Avian Influenza Control and Pandemic Influenza Preparedness) 2006-2008.

Several activities that were increased, in part:

*) developed 8 regional diagnostic laboratories and the Litbangkes Body laboratory to BSL-3 (Bio Safety Level 3), to check the living virus.

*) continued the socialisation of the policy and the intensification penatalaksanaan the case as well as the speed of case reconciliation.

*) reinforced Early Warning System (the early detection) and Surveilans. Equipped the intensive maintenance equipment in 100 reconciliation hospitals.

*) intensified risk communication in developing the awareness of the social stratum.

*) developed the alert village in the health field including the prevention and the FB control.

*) developed "Pilot Project" the prevention and the FB control in Tangerang as the model, co-operated with the Singaporean government.

*) reinforced the co-ordination of the sector passage especially with the Department of Agriculture that was competent in the handling of the source of the infection in the poultry.

*) the epidemiology Research, clinical, the environment and virologis. *) produced Oseltamivir domestic and developed the based vaccine strain H5N1 Indonesia and rapid test to detect the FB virus to humankind, co-operating in a foreign manner. Moreover, also had the Plan.

The price of Generic Medicine will rise?

By editorial staff of 20/10/2008 @ 23:49 Surabaya,

on Tuesday — Menkes Siti Fadilah Supari said, at this time the government was reviewing the price of the generic medicine and the import of the medicine following the financial crisis global. The "price" of "generic medicine has been determined by me and because of the existence" of the "rise US dollar, of course if indeed his difference was very big we will make one adjustment that certainly did not make the people heavier," said Menkes in Surabaya, on Tuesday. It was further, he says, "Now we were counting whether must be adapted or not." If necessary, how many and whether being placed to the people or must we susbsidi.

Apart from the price of generic medicine, the planned also government restricted the import of foreign medicines of the company's patent that will make the balance of the country's finance heavier. "According to me must be considered in being studied by us repeated." If needing to be able to not have the import of medicines and we for generic medicine here, where generic could be produced by the domestic company and the price him could be arranged, he said. Nurliga's Father Unhappy With Treatment
March 3, 2009
http://news.okezone.com/index.php/ReadStory/2009/03/02/1/197666/bhakti-yudha-salah-diagnosa-pasien-suspect-flu-burung
Depok: The family of Nurliga Mahmudin (5), a bird flu suspect casualty, regretted the diagnosis mistake and indolent handling from RS Bhakti Yudha Depok. According to the casualties's father, Mahmudin Sudin, the doctor diagnosed the wrong illness and if faster action had been carried out possibly Nurliga could have still been rescued.

"I thought this diagnosis was wrong."

After being treated since Monday last week, results of this diagnosis were made Wednesday and was stated by DBD, explained Mahmudin Sudin in his residence at Kp Pulo RW 6 RT 8 Rangkapan Jaya districts, Depok, on Monday (2/3/2009). However on Wednesday night, the patient experienced breathlessness that increasingly worsened so as to be stated that he was affected by the inflammation of the lungs.

"Initially the family agreed, and the patient was given cough medicine to expedite phlegm. Then he was given oxygen as well as having fumigation to his lungs," explained Mahmudin who is also Chairman of the Depok Honorer Teacher's Association.

His condition continued to worsen, then the medical team of RS Bhakti Yudha put the casualty in the Case Treatment Unit (UPK). This was considered so that the restoration of the casualty's health would be more sterile. In the meantime from results of the x-ray photographs there were white flecks that covered all of Nurliga's lung. At approximately 17,00 WIB, hospital personnel could not handle him and decided to reconcile the casualty to RS Persahabatan. The family really regretted the slow work of RS Bhakti Yudha when reconciling the bird flu suspect casualty to RS Persahabatan.

He explained that initially they were promised a special car to bring the patient to RS Persahabatan. While the doctor was busy contacting 116 to ask for the ambulance, finally the car from RS Bhakti Yuda also was used, because the ambulance from Diskes Depok never arrived. The result was that the casualty did not immediately receive the exact maintenance. Upon arriving in RS Persahabatan his condition was serious. Almost all the space on his lungs were filled with white spots that increasingly spread like clouds in the sky.

Mahmudin also suspected RS Bhakti Yudha of making his child a guinea pig, because they took so many samples from several of parts of the child's body. According to him, the family must cover the cost of the maintenance for five days in RS Bhakti Yudha as much as Rp3 million more, whereas in RS Persahabatan it is free because of entering the national bird flu handling program. In the meantime Depok Mayor Nur Mahmudi Ismail with his rank in Dinkes, Distan and Angota DPRD paid a condolence call on the family for the casualty's death. The casualties's father, who is also Depok religious teacher SMAN 1, added that the blood samples have been taken from the hospital and currently are being researched in the laboratory belonging to IPB Bogor. As for the chicken coop that was suspected as the place that infected him with the illness resulting from the virus H2N5, it has been sprayed with disinfectant by Distan Depok. His body already dikemumikan last Saturday, February 28 in TPU Aisyiah or to be precise 300 metres from the casualty's house by using the special case from RS Persahatan.

4 Indonesians die of bird flu; toll climbs to 119
http://www.thejakartapost.com/news/2009/03/03/4-indonesians-die-bird-flu-toll-climbs-119.html
March 3, 2009
Irwan Firdaus , Associated Press , Jakarta | Four Indonesians have died of bird flu, bringing the death toll in the country hardest hit by the disease to 119, an official said Tuesday. Bayu Krisnamurthi, chief of the National Bird Flu Commission, said all the victims were believed to have been infected after coming into contact with sick poultry. They were from Java island and died in January and February.

Indonesia, which has been criticized in recent months for refusing to immediately make public news of human deaths and in some cases hushing them up all together, has been worst affected by bird flu since it started ravaging Asian poultry stocks in late 2003. It accounts for nearly half the 256 human fatalities tallied worldwide, according to the World Health Organization.

So far, most human cases have been linked to contact with infected chickens. But health experts worry the H5N1 virus could mutate into a form that passes easily among humans, sparking a pandemic that could kill millions worldwide. Indonesia is considered a potential hotspot for that to happen.

Krisnamurthi had little details about the latest deaths, saying only that two were siblings from the city of Bogor and the others were women from Bekasi and Surabaya. Health Minister Siti Fadilah Supari has withheld almost all virus samples from WHO since January 2007, arguing that its system is unfair because her country's bird flu specimens can be used to make vaccine that would ultimately be too expensive for most Indonesians and controlled by wealthy nations. She has since called for the creation of a global stockpile of drugs or other forms of benefit-sharing.

http://www.earthtimes.org/articles/show/258227,bird-flu-kills-four-this-year-in-indonesia-death-toll-119.html
March 3, 2009
Jakarta: Four Indonesians have died of bird flu this year, bringing the national death toll from the disease to 119, an official said Tuesday. Three of the victims were from West Java province while the other lived in East Java, said the official, who declined to be identified because the person was not authorized to speak to the media. The official declined to give further details.

Indonesia has the highest death toll in the world from bird flu. Since last year Indonesia has stopped announcing bird flu deaths on a case-by-case basis and instead reported them every few months. The most common way to contract the virus is through human contact with infected fowl. Bird flu remains chiefly an animal disease, but experts fear the virus could mutate into a form that could transmit between humans and spark a pandemic that could kill millions around the world.

The Bird Flu Handling Team Monitors 33 People
Tuesday, March 03, 2009, 19:53:00
BEKASI: The case of bird flu in County Bekasi continued after a child died, Ardiansyah (Cool. The West Java County Bekasi Bird Flu Team said that a total of 33 people in the Sukamantri Village, Desa Suka Raya, Subdistrict Karang Bahagia, received special supervision.

"They were involved in close contact of less than one meter from where the victim died," said one of the team members of County Bekaris Bird Flu Handling Team, Heru Hermawan, in the location of the bird flu incident, on Tuesday (3 / 3).

The majority of the 33 people are relatives of Ardiansyah and some medical staff who had cared for the dead child. Family members are Ilyas (43), Mimin Setiawati (36), Ismawati (4), and Nanang. The medical staff includes clinic owners Kartika Medika, Ipah Latifa and Tarmizi in a house not far from the victim. Heru said that the supervision is done twice during an incubation period of 17 days from the day that Ardiansyah died.

"The goal is to prevent the transmission of avian influenza virus type H5N1 from poultry to humans and from humans to humans. Just because one complains about having the pain like a sick chicken with bird flu, including fever, coughing and congestion, they are not necessarily bird flu positive," said Heru.

The spread of bird flu to poultry in 2009 has occurred in two areas, Subdistrict Sukatani and Subdistrict Setu. The causes, said Heru, is there is not good quality governance for birds. Chickens kept in the wild and penned with sticks to the walls of the house are left dirty.

"The number of attacks on birds in the area that high," said Heru.
